Bookkeeping service agreement form

Fee is $ 200 for the first 50 transactions recorded by us and $ 0.65 each thereafter to record all your transactions that include all bills, invoices, checks written, cash withdrawals, and bank deposits made by you. Work begins as soon as you start sending us the data. This agreement is cancelable by both you and A Andrew Harrison CPA PC after giving written notice. This agreement is cancelable for failure to pay the required fees. Please fill the form below to start this service.

Please note: This service includes a free payroll service. Just call in, fax or e-mail your pay checks gross amount and we will keep track of your federal, state and city withholding and tell you how much to deposit with your bank and your net take home pay. You have to choose a weekly, biweekly, or semimonthly payroll period and fill out an IRS form W-4 Withholding allowance for the required exemption amount(s).

This service is not for the preparation of tax forms (W-2, W-4, 941, 940, 1040, 1099, 1120, etc.) or the preparation of financial statements. Please use the correct service agreement forms located on our website.

Please call in, fax or e-mail your bills on a weekly basis and we will record them in our computerized bookkeeping system. Financial statements will be prepared at your request from the data we have recorded. (Our fax number 1-718-363-9500 is the same as our regular number. You have to call us to tell us to activate the fax feature).
